During the first visit, we will review important information like your medical history and give you time to get to know your doctor. We accept most insurances. Dr. Sellers-Hannibal and her congenial staff look forward to helping you feel better! New patients including children are welcomed here. It would be a pleasure to soothe your sole step by step!
Patient Forms
Please print and fill out the New Patient forms so we can expedite your first visit.
New PatientsPlease arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Cancellations
There is a $50 cancellation fee for any appointments not cancelled within 24 hours of the scheduled appointment.
Holidays
This office is closed in observance of the following holidays:
- New Year's Day
- Memorial Day
- Juneteenth
- Independence Day/July 4th
- Labor Day
- Thanksgiving
- Christmas Day
There are limited hours on New Year's Eve, the day prior to Thanksgiving, Christmas Eve, and the last week in December.
